POWER SHOWCASE Photo and Video Release and

Authorization for Use of Photo and Video by Individuals

I hereby grant to and authorize Power Showcase, Inc, its employees, successors in interest, licensees, assigns and agents, the right to take, use, reproduce, exhibit, display, broadcast, distribute and create derivative works of visual/audio images of me and my name, likeness and biographical information in connection with all activities of Power Showcase, Inc. or for promoting, publicizing or explaining Power Showcase, Inc., its events and activities. Visual/audio images are any type of recording, including but not limited to photographs, digital images, drawings, renderings, voices, sounds, interviews, video recordings, audio clips or accompanying written descriptions. I agree that Power Showcase, Inc. owns the images and all rights related to them.

I do hereby give Power Showcase, Inc., its employees, successors in interest, licensees, assigns, and agents, the irrevocable right to use the visual/audio images of me and my name, likeness and biographical information in any manner or media now known or later invented including but not limited to Power Showcase sponsored websites, publications, promotions, broadcasts, and advertisements. I waive any right to inspect or approve the finished video/audio images or any printed or electronic matter including the video/audio images or my name, likeness or biographical information, or to be compensated for use of such.  .

I hereby release Power Showcase, Inc., its employees, successors in interest, licensees, assigns and agents from any claims, damages or liability which I may ever have in connection with the taking or use of the images or printed material including the video/audio images, my name, likeness and biographical information.  .

I am at least 18 years of age and am competent sign this release . I have read this release before signing below and I fully understand the contents, meaning, and impact of this release and I freely accept the terms.
